About Grace Community Outreach
Grace Community Outreach Inc. is a 501(c)(3) nonprofit organization committed to meeting the basic human needs of individuals and families in underserved communities. Our mission is to empower these communities by providing essential services and opportunities that promote well-being and support a path toward a better life.
We offer a variety of support services, including food, clothing, home goods, counseling, and transportation assistance. Additionally, we provide comprehensive programs focused on healthcare, senior support, reentry services, mental health, health education, technology access, after-school enrichment, and entrepreneurship.

Objectives
1. Collaborate with Key Stakeholders
Partner with other nonprofits, community leaders, healthcare professionals, government agencies, and corporations to comprehensively address healthcare needs and improve the living conditions of underserved populations.
2. Address Basic Needs through Distribution Programs
Support food distribution programs and provide home goods, appliances, and clothing to meet basic needs, stabilize lives, and enable individuals and families to focus on personal growth and well-being.
3. Provide Access to Essential Resources
Provide resources and facilities that address the social determinants of health by establishing support groups, enhancing access to technology, delivering education, and securing funding to meet critical needs.
4. Expand Telehealth Services
Ensure that therapy and doctor visits are accessible through a telehealth center, particularly for those hindered by unreliable transportation and distance. Raise resources to establish telehealth centers in community hubs like churches, schools, senior living centers, libraries, and at-risk apartment buildings.
5. Develop Strategic Partnerships
Forge connections with schools, other nonprofits, social services, and the penal system to support vulnerable individuals and address unique challenges faced by these populations.
6. Healthcare Advocacy
Offer educational programs, support groups and healthcare navigational services to assist individuals to access their healthcare options and understand how to use their healthcare plans.
