Grace Community Outreach Inc. is a 501(c)(3) nonprofit organization committed to meeting the basic human needs of individuals and families in underserved communities. Our mission is to empower these communities by providing essential services and opportunities that promote well-being and support a path toward a better life.
We offer a variety of support services, including food, clothing, home goods, counseling, and transportation assistance. Additionally, we provide comprehensive programs focused on healthcare, senior support, reentry services, mental health, health education, technology access, after-school enrichment, and entrepreneurship.

Process
Register your practice, Complete Short Application and participate in 30-minute interview.
Cost
The cost is 30 minutes of your time monthly to answer general questions about patient engagement, and the type of support needed for patients. No specific questions about a patient’s health situation.
Criteria
Must be a state licensed MD, DO, Nurse Practitioner, Physicians Assistant or Certified mental health professional.
